Access Cards

Please carefully read the agreement, and then complete the form below. Once you have completed the form, you will either be sent an invoice to make the deposit on-line, or you may come into the office to leave your deposit, as you choose. Your card request will be processed as soon as possible, but please expect it to take at least one day. Same-day pick ups are possible, but not guaranteed.


Access Card Agreement

This Access Card Agreement (hereinafter referred to as the “Agreement”) is entered into between  (hereinafter referred to as the “Cardholder”) and Canadian Martyrs Church and Saint Kateri Student Chaplaincy (hereinafter referred to as the “Church”). The Agreement outlines the terms and conditions regarding the use and return of access cards to the student centre. By accepting and using the access card, the Cardholder agrees to abide by the following terms:

  1. Electronic Monitoring and Usage Review: The Cardholder acknowledges that electronic access to the student centre will be monitored and logged. The administration reserves the right to review usage reports and logs for security and administrative purposes.

  2. Limited Access Hours: Access to the student centre will be limited to established open hours as determined by the administration. The Cardholder agrees to respect and adhere to these designated hours.

  3. After-Hours Access: After-hours access to the student centre is intended for chaplaincy-related activities only. Permission for after-hours access will be granted on a case-by-case basis by the administration.

  4. Ownership and Return of Access Cards: The access cards issued to the Cardholder remain the property of Canadian Martyrs Church and Saint Kateri Student Chaplaincy. The Cardholder agrees to return the access card to the administration upon request or at the end of the academic year (May), unless permission has been granted to keep the card over the summer, or if the Cardholder no longer holds the role, responsibilities, and/or position for which the access card was granted.

  5. Refundable Deposit: A refundable deposit of $20 is required for the issuance of the access card. This deposit will be returned to the Cardholder upon the return of the access card to the administration.

  6. Prohibited Actions: The Cardholder agrees not to loan, transfer, give possession of, misuse, modify, alter, or make a copy of the access card. Any violation of this provision may result in the revocation of access privileges and appropriate disciplinary action.

  7. Security Measures: The Cardholder agrees to take necessary security measures and understands that they are never to prop open the door or leave it unsecured in any way. It is the responsibility of the Cardholder to ensure that the student centre is securely locked when leaving the premises.

  8. Intended Use of Space: The Cardholder acknowledges and understands that the student centre is primarily intended for fellowship and chaplaincy-related activities. There is no expectation that the space will be made available for, or designated as a study area. The Cardholder agrees to respect and adhere to this intended use of the space.

  9. Furniture and Furnishing: The Cardholder agrees to respect the arrangement of furniture and furnishing within the student centre and will ensure that they are returned to their original placement after use. Any adjustments made for specific activities must be restored to the original configuration upon completion. This includes chairs, tables, equipment, and any other items within the student centre.

  10. Cleanliness and Respect for Space: The Cardholder agrees to keep the student centre and specifically the kitchen area clean and tidy during their use of the facility. The Cardholder will clean up after themselves, properly wash and put away any dishes, utensils, or equipment used, and ensure that all surfaces are wiped clean after use. It is important to maintain a high standard of cleanliness and hygiene in the kitchen area, considering the preparation and storage of food.

Failure to abide by the above stipulations will result in the revocation of access to the student centre.
